Can’t make it to the MN but still want to raise money for the Liz Logelin Foundation 5k? No problem! Just download a registration and donation form and start fundraising! And if you get your forms in by September 4th, 2009, you’ll still get your t-shirt in time for the walk! (For international participants, please email Becky for t-shirt shipping details.)

Please print, read through and sign this form and return it to the listed address, and you’re set to plan your own walk!
